The Philippine Embassy in Rome would like to inform all Filipinos in Italy that the Italian Ministry of Interior has issued a revised Autodichiarazione or Self-Declaration form in connection with the new Decree of the President of the Council of Ministers (DPCM) signed on 25 March 2020, providing additional measures to contain the COVID-19 outbreak.

In the new form, the region of departure and region of arrival must be indicated together with the purpose of travel, which should be included in those that are allowed under the new decree.

Filipinos are reminded to declare accurate information as police authorities could verify the accuracy of the information provided.

Non-compliance with these regulations may result to penalty or imprisonment.
