Applicants for RENEWAL of Philippine e-passports will now have the OPTION to apply either at the Philippine Embassy in Rome (https://romepe.timetap.com) or at the BLS Philippine Passport Application Centre (https://blsphilippinepassport.com).

The Philippine Embassy in Rome will exclusively process the following applications:

  1. Applications for new passports
  2. Replacement of lost passports
  3. Applications requiring changes in the biographical details of passport holders (such as changes in name, middle name, surname, civil status, date of birth, place of birth, gender).

 

Adult e-Passport Renewal

General Requirements:       

    1. Confirmed appointment
    2. Personal appearance of the applicant
    3. Duly accomplished application form (click here to download the form)
    4. Current Passport with photocopy of data page
    5. Original and photocopy (back-to-back) of valid Italian Carta d’Identita or Permesso di Soggiorno or any valid Philippine government-issued ID

Additional Requirements:

For changes in passport entries (e.g. spelling of name, place of birth, etc.)

  • Original and photocopy of Birth Certificate on security paper issued by the Philippine Statistics Authority (PSA)

For dual citizens

  • Original and photocopy of Identification Certificate issued by the Philippine Embassy

For married females

  • Original and photocopy of Marriage Certificate on security paper issued by the Philippine Statistics Authority

For married females who want to revert to their single name

  • Original and photocopy of Marriage Certificate on security paper with annotation of annulment/divorce/court ordered instruction issued by the Philippine Statistics Authority (PSA), if applicable
  • Original and photocopy of Birth Certificate on security paper issued by the Philippine Statistics Authority (PSA) and original and photocopy of Death Certificate of Spouse on security paper issued by the Philippine Statistics Authority (PSA), if applicable

Adult Lost e-Passport

General Requirements:

  2. Personal apperance of the applicant
  3. Duly accomplished application form (click here to download the form)
  4. Original and photocopy (back-to-back) of valid Italian Carta d’Identita or Permesso di Soggiorno or any valid Philippine government-issued ID
  5. Original and photocopy of Birth Certificate on security paper issued by the Philippine Statistics Authority (PSA)
  6. Affidavit of Loss
  7. Police Report (Denuncia) with English translation

Additional Requirements:

For dual citizens

  • Original and photocopy of Identification Certificate issued by the Philippine Embassy

For married females

  • Original and photocopy of Marriage Certificate on security paper issued by the Philippine Statistics Authority

For married females who want to revert to their single name

  • Original and photocopy of Marriage Certificate on security paper with annotation of annulment/divorce/court ordered instruction issued by the Philippine Statistics Authority (PSA), if applicable
  • Original and photocopy of Birth Certificate on security paper issued by the Philippine Statistics Authority (PSA) and original and photocopy of Death Certificate of Spouse on security paper issued by the Philippine Statistics Authority (PSA), if applicable

Note: A Police Report (denuncia) is not required if the lost passport is already expired at the time of application for renewal of lost passport.

Adult Non-e-Passport Renewal (Brown, Green, Machine Readable (Maroon) Passports or any older Passport) bearing no middle name

General Requirements:

Minor e-Passport Renewal (Below 18 years old)

General Requirements:

  2. Personal appearance of the applicant AND either parent or authorized adult companion.
  3. Duly accomplished application form (click here to download the form)
  4. Current Passport with photocopy of data page
  5. Original and photocopy of Birth Certificate on Security Paper issued by the Philippine Statistics Authority (PSA) or Report of Birth issued by the Philippine Embassy
  6. Original and photocopy (back-to-back) of valid Italian Carta d’Identita or Permesso di Soggiorno or any valid Philippine government-issued ID
  7. Original and photocopy of Passport and valid ID of either parent.

Additional Requirements:

For legitimate minors accompanied only by the father:

  • Original and photocopy of Marriage Certificate on security paper issued by the Philippine Statistics Authority (PSA) or Report of Marriage issued by the Philippine Embassy.

For minors who are derivatives of dual citizen parent/s:

  • Original and photocopy of Identification Certificate of the dual citizen parent/s issued by the Philippine Embassy.

New e-Passport Application for Minors (Below 18 years old)

General Requirements:

  4. Original and photocopy of Birth Certificate on security paper issued by the Philippine Statistics Authority (PSA) or Report of Birth issued by the Philippine Embassy
  5. Original and photocopy (back-to-back) of valid Italian Carta d’Identita or Permesso di Soggiorno or any valid Philippine government-issued ID
  6. Original and photocopy of Passport or valid ID of either parent, e.g.,  Permesso di Soggiorno or Carta d’Identita.

Additional Requirements:

For new-born applicants

  • Application for Report of Birth (click here for the requirements)

For legitimate minors accompanied only by the father:

  • Original and photocopy of Marriage Certificate on security paper issued by the Philippine Statistics Authority (PSA) or Report of Marriage issued by the Philippine Embassy.

For minors who are derivatives of dual citizen parent/s:

  • Original and photocopy of Identification Certificate of the dual citizen parent/s issued by the Philippine Embassy.

  • Social Security System (SSS) or Government Service Insurance System (GSIS) Unified Multi-Purpose Identification (UMID) Card
  • Land Transportation Office (LTO) Driver's License
  • Professional Regulatory Commission (PRC) ID
  • Overseas Workers Welfare Administration (OWWA) or Integrated Department of Labor and Employment (iDOLE) card
  • Commission on Elections (COMELEC) Voter's ID or Voter's Registration Record from COMELEC Head or Regional Office
  • Philippine National Police (PNP) Firearms License
  • Senior Citizen ID
  • Persons with Disabilities (PWD) ID
  • School ID (for students)

Reminders on Passport Releasing

  • The passport is usually released within 4 to 6 weeks after the date of application.
  • Applicants may check the availability of their passports through the Embassy’s website (http://romepe.dfa.gov.ph) or Facebook page (https://www.facebook.com/PHinItaly).
  • The Embassy’s Releasing Section is open from Mondays to Fridays, 8:30am to 5:00pm.
  • Applicants should present their old passport and original receipt to claim their new passport.
  • For applicants who cannot claim their new passports themselves, their representatives must also submit an authorization letter and a valid ID.
  • Applicants who are thirteen (13) years old and above must sign the third page of the passport. Applicants below thirteen (13) years old need not sign the third page of the passport.
  • Passports unclaimed after 6 months from the date of issue shall be cancelled.
  • Passports of applicants who applied during a consular outreach will be released at the Philippine Honorary Consulate near the outreach venue.
