For Certificates for presentation to Italian authorities in southern and central Italy, secure the same from the Philippine Embassy in Rome.
For Certificates for presentation to Italian authorities in northern Italy, secure the same from the Philippine Consulate General in Milan (https://milanpcg.dfa.gov.ph/)
Requirements:
- Appointment
- Personal Appearance of the client
- Passport (1 original, 1 photocopy) of the applicant
- Written request/Salaysay, stating the reason behind the request and for what purpose it will be used.
- Permesso di soggiorno or Carta d’identità (original & photocopy)

A. CERTIFICATE OF ONE AND THE SAME PERSON:
- Birth Certificate PSA issued (original & photocopy)
- Other Italian documents with discrepancies in the applicant’s names (Original & photocopy)
- Other Philippine documents with discrepancies in the applicant’s names (Original & photocopy)
B. CERTIFICATE ON CONTENTS OF BIRTH CERTIFICATE
- PSA issued-Birth Certificate (original & photocopy)
C. CERTIFICATE ON CONTENTS OF MARRIAGE CERTIFICATE
- Marriage Certificate PSA issued (original & photocopy)
D. CERTIFICATE ON CONTENTS OF DEATH CERTIFICATE
- PSA issued-Death Certificate (original & photocopy)
E. CERTIFICATE ON FAMILY COMPOSITION/STATO DI FAMIGLIA

For non-direct family members:

- PSA issued-Birth Certificate (original & photocopy)/Marriage Certificate (to verify family connection/affiliation)
F. CERTIFICATE OF NO INCOME/PROPERTY (for PENSION)
- Affidavit stating that client has no income/property in the Philippines (statement can be added to the written request)
- PSA-issued Birth Certificate (original & photocopy)
- Certificate of No Income issued by the BIR (requested at the municipality of the client’s residence in the Phils.)
- Certificate of No Property issued by the Registry of Deeds (requested at the municipality of the client’s residence in the Phils.)
- Certificate of No Pension issued by the SSS office at the Embassy
G. CERTIFICATE FOR TAX DECLARATION 730/REQUEST FOR CODICE FISCALE OF DEPENDENTS FOR TAX EXEMPTION/ ASSEGNO FAMILIARE
- PSA-issued Birth Certificate (original & photocopy)
- Affidavit of the client stating the reason behind the request and indicate that her dependents have no property, income and or pension in the Phils; if seniors/adults)
Additional requirements:
For minor dependents/adult dependents studying:
- Philippine Passport (original & photocopy)
- PSA-issued Birth Certificate (original & photocopy)
- School or University IDs (scanned copies not printed images taken by a photo camera) or proof of enrolment for the scholastic year (scanned copies)
For adult dependents:
- Philippine Passport w/ signature (original & photocopy)
- PSA-issued Birth Certificate (original & photocopy)
- Certificate of No Income issued by the BIR (requested at the municipality of the client’s residence in the Phils.)
For senior citizen dependents:
- Philippine Passport w/ signature (original & photocopy)
- PSA-issued (Birth Certificate original & photocopy)
- Senior Citizen’s ID (scanned copy)
Processing Fees:
- Application Fee of € 27.50 (for each set of documents)
- Expedite Fee of € 11.00 (for same day release of document)
Reminders:
- Applicant must be an adult (18 years old and above and with legal capacity).

- Applicant with incomplete supporting documentary requirements will not be processed.
- The Consular Officer reserves the right to require additional documents upon assessment of the application, pursuant to relevant laws, rules and regulations.
