The Philippine Embassy in Rome would like to inform all Filipinos in Italy that the Italian Ministry of Interior has issued a revised Autodichiarazione or Self-Declaration form for movements allowed by the Italian authorities such as for proven work needs, health reasons and for necessities.
The new form includes a declaration that the person signing is not under quarantine and is not positive of COVID-19.
The new form also requires the police to countersign the self-declaration, certifying that it was made in his presence and the person has presented a valid identification document. In this case, the person no longer needs to attach a photocopy of his/her identification document to the self-declaration.
Filipinos are reminded to declare accurate information as police authorities could verify the accuracy of the information provided.
Non-compliance with these regulations may result to a penalty of Euro 206 or 3 months imprisonment.
