REPORT OF BIRTH

The birth of a child born in Italy or other countries under the jurisdiction of the Philippine Embassy in Rome to parents who are both Filipino citizens or to one parent who is a Filipino citizen must immediately be reported to the Philippine Embassy in Rome through the application for Report of Birth (ROB).

The ROB will be forwarded to the Department of Foreign Affairs (DFA) every end of the month for onward transmittal to the Philippine Statistics Authority (PSA). The transmittal of the ROB will be indicated in the parents’ copy of the ROB, for their reference.

Please note the rule that only information obtained at the time of birth shall be recorded in the birth certificate, and not the information prevailing at the time of registration, as per Manual of Instructions of R.A. 9048. In view of this rule, parents who are already married at the time of registration, but whose child was born before their marriage, cannot register their child as legitimate. Therefore, parents applying for ROB of their children born before these parents were married (e.g. child was born on 15 February 1995 and parents of said child got married only on 15 March 1995) shall submit the required documents.

For a Legitimate Child

General Requirements:

  1. Personal appearance of the one of the parents
  2. Four (4) copies of Duly Accomplished Report of Birth Application Form signed by one of the parents
  3. Estratto Di Nascita issued by the Commune where the child was born, indicating the child’s date and place of birth, and names of both parents - original & four (4) photocopies
  4. Hospital record indicating time of birth of the child - original & four (4) photocopies
  5. Marriage Certificate or Report of Marriage on security paper issued by the Philippine Statistics Authority (PSA) – original & four (4) photocopies
  6. Passport of both parents - original & four (4) photocopies

Additional Requirements:

For Late Registration of Birth (the child is already more than one (1) year old at the time of registration of the birth)

  1. Affidavit of Delayed Registration
  1. Negative Report of Birth (original) on security paper issued by the Philippine Statistics Authority in Manila (valid for one year upon registration)

For an Illegitimate Child

General Requirements:

  1. Personal appearance of the one of the parents
  2. Four (4) copies of Duly Accomplished Report of Birth Application Form signed by one of the parents
  3. Estratto Di Nascita issued by the Commune where the child was born, indicating the child’s date and place of birth, and names of both parents - original & four (4) photocopies
  4. Hospital record indicating time of birth of the child - original & four (4) photocopies
  5. Marriage Certificate or Report of Marriage on security paper issued by the Philippine Statistics Authority (PSA) – original & four (4) photocopies
  6. Passport of both parents - original & four (4) photocopies
  7. Affidavit to Use Surname of Father (AUSF) in four (4) originals. A copy of the passport of the person executing the affidavit will be attached to the AUSF.
    1. If the child is six (6) years old or below, the mother or, in her absence, the legal guardian, shall execute the  AUSF
    2. If the child is seven (7) years old or above up to seventeen (17) years old, the child shall execute the AUSF with an accompanying attestation of the mother or the legal guardian.
    3. If the person is eighteen years (18) old or above, the person shall execute the AUSF without an attestation of the mother or legal guardian.
  8. Affidavit of Acknowledgement of Paternity (AAP) in four (4) originals or Private Handwritten Instrument (PHI) in one (1) original and three (3) certified true copies (CTCs). Attached to each AAP or PHI is a copy of the father’s passport.
    1. The father shall execute the AAP.
    2. If the whereabouts of the father is unknown, the mother, legal guardian or the adult person himself/herself shall present the original PHI. The PHI shall be supported by an Affidavit of Explanation to be executed by the mother or legal guardian.
    3. If the father is deceased, the original Death Certificate shall be required.
  9. Certificate of Registration for requirements 7 and 8.
  10. Other documents that the Embassy deem necessary to support the registration of the birth of the child.

Additional Requirements:
